Bakgrund: Antalet äldre i Sverige och världen ökar konstant. För att uppnå en god vård för äldre med komplexa vårdbehov i hemmet krävs ett bra samarbete mellan regionen och kommunen. I västra Östergötland finns ett mobilt team, ALMA-teamet, som utgår ifrån lasarettet i Motala. Teamet består av sjuksköterskor och läkare som arbetar för att förbättra vården för äldre med komplexa vårdbehov. Syfte: Syftet med studien var att beskriva sjuksköterskors/distriktsköterskors erfarenhet av samarbetet mellan hemsjukvården, ALMA-teamet och primärvården i Västra Östergötland. Metod: En kvalitativ metod med induktiv ansats användes, 13 intervjuer genomfördes med sjuksköterskor/distriktsköterskor inom hemsjukvården och ALMA-teamet. Innehållsanalys användes vid bearbetning av data. Resultat: Resultatets huvudfynd visar att samarbetet mellan ALMA-teamet och hemsjukvården fungerar bra och att samarbetet mellan hemsjukvården och primärvården fungerade mindre bra, sett ur hemsjukvårdens synvinkel. Slutsats: Ett välfungerande samarbete mellan hemsjukvården och ALMA-teamet leder till en bättre vård för äldre med komplexa vårdbehov. Att samarbetet brister mellan hemsjukvården och primärvården beror på bristande kunskap om varandras resurser och rutiner. ALMA-teamets arbetssätt skulle kunna appliceras på andra regioner. Background: The number of elderly people in Sweden and the world is constantly increasing. In order to achieve good care for the elderly with complex care needs in the home, good cooperation between the region and the municipality is required. In western Östergötland there is a mobile team, the ALMA team, which is based on the hospital in Motala. The team consists of nurses and doctors who work to improve care for the elderly with complex care needs. Aim: The purpose of the study was to describe nurses '/ district nurses' experience of the collaboration between home health care, the ALMA team and primary care in Western Östergötland. Method: A qualitative method with inductive approach was used, 13 interviews were conducted with nurses / district nurses within the home healthcare and ALMA team. Content analysis was used in data processing. Result: The main findings of the result show that the collaboration between the ALMA team and the home health services worked well and that the cooperation between the home health care and the primary care functioned less well, seen from the home health care perspective. Conclusion: A well-functioning collaboration between home health care and the ALMA team leads to better care for the elderly with complex care needs. The lack of cooperation between home health care and primary care is due to a lack of knowledge of each other's resources and routines.
