Patient’s experience of palliative care at home : A qualitative literature review (English)
Abstract [sv]

Bakgrund:

Abstract [en]

Main title: Patient’s experience of palliative care at home: A qualitative literature review

Background: Palliative care aims to relieve symptoms and improve quality of life. Palliative care is divided into two phases: an early phase and a late terminal phase. Break point conversation takes place when transitioning to the late terminal phase. The four main pillars of palliative care are symptom management, support for the patient’s relatives, teamwork and communication. Many patients preferred to spend their last time at home with their loved ones where the environment felt familiar. The nurse plays a key role in ensuring good and person-centered palliative care.

Aim: The purpose of this study was to describe patients' experiences of palliative care at home.

Method: A qualitative literature review was conducted where 13 qualitative articles were included and analyzed according to Friberg's thematic model.

Results: The thematic analysis resulted in three main themes: “Home as a care environment” with subthemes that describe the positive and negative aspects of home care, “The caring encounter at home” that describes patients' insights about communication, trust, and the multi-professional care-team, “pre-conditions for good palliative care at home" that illuminates the importance of seeing the person as a whole and patients participation in their own care.

Conclusion: The literature review shows that receiving palliative care at home was perceived as positive and reassuring by the patients. A care relationship built on trust and good communication between the patient and healthcare professionals improved the patient’s well-being. A holistic approach and patient participation in care are important elements to achieve good palliative care.
