Bakgrund: I Sverige beräknas ca 1,5% av befolkningen ha diagnosen intellektuell funktionsnedsättning. Diagnosen innefattar påverkan på flera förmågor inklusive kommunikativa förmågor vilket tar sig uttryck i bland annat svårigheter med verbal kommunikation. Patienter med intellektuell funktionsnedsättning upplever ofta stress och oro i samband med vårdbesök och har större behov av vård än den generella befolkningen. Sjuksköterskan ansvarar för att ge individuellt anpassad vård och skapa delaktighet utifrån ett personcentrerat förhållningsätt. Syfte: Syftet var att belysa sjuksköterskors erfarenheter av kommunikation med patienter med intellektuell funktionsnedsättning. Metod: Kvalitativ litteraturöversikt med induktiv ansats utifrån tretton vetenskapliga artiklar. Artiklarna analyserades utifrån Fribergs (2022) fem steg för integrerad sammanställning. Resultat: Två teman med tillhörande fem subteman identifierades: Relationers betydelse vid kommunikation (sjuksköterskans förhållningsätt och attityder, att lära känna patienten, närståendes roll) samt alternativa kommunikationsformer (användning av hjälpmedel vid kommunikation, icke verbal kommunikation). I resultatet framkom vikten av att bygga relationer med patienter och närstående samt vikten av att vara anpassningsbar vid kommunikation. Även sjuksköterskors förhållningsätt och attityder påverkade kommunikationen. Slutsats: Kommunikation ansågs vara en förutsättning för att ge säker vård av hög kvalité. Det framkommer behov av ökad kunskap kring både diagnosen samt kommunikationsformer för att effektiv kommunikation ska uppnås.
Background: About 1,5% of the population in Sweden is estimated to have the diagnosis of intellectual disability. Part of the diagnosis affects the ability to communicate which can express itself in difficulty with verbal communication. Patients with intellectual disability often experience stress and anxiety related to healthcare visits and have a higher need for healthcare than the general population. The nurse is responsible for delivering personalized care and creating patient participation based on a person- centered approach. Aim: The purpose was to illustrate nurses’ experiences of communication with patients with intellectual disabilities. Method: Qualitative literature review with an indictive approach based on thirteen articles. The articles were analyzed according to Friberg in five steps. Results: Two themes with five subthemes were identified: The meaning of relationships in communication (nurses approach and attitudes, to get to know the patient, the role of family and caregivers) and alternative forms of communication (use of tools in communication, non-verbal communication). The result highlights the importance of building relationships with family and caregivers as well as the importance of adaptability when communicating. The nurses’ views and attitudes were also shown to have an impact on communication. Conclusion: Communication was considered a prerequisite to ensure safe care of high quality. A need for increased knowledge of both the diagnosis as well as different forms of communication were also identified to achieve effective communication.
