Sammanfattning 

Bakgrund: En frisk åldrande befolkning medför att samhället får lättare att stödja och vårda äldre. Hälsosamtal riktade till personer upp till 70 år finns som insats på vårdcentraler i flera svenska regioner. Hälsofrämjande arbete med äldre över 80 år har inte samma struktur. Vården skulle kunna stödja till hälsa ur många aspekter genom hälsofrämjande arbete och personcentrerat förhållningssätt.

Syfte: Syftet var att undersöka distriktssköterskors erfarenheter kring att arbeta hälsofrämjande med äldre över 80 år, inom primärvårdens verksamhet. 

Metod: En kvalitativ studie med induktiv ansats enligt Graneheim och Lundman genomfördes.  Genom ett ändamålsenligt urval deltog tolv distriktssköterskor från södra Sverige i semistrukturerade intervjuer under hösten 2024.  

Resultat: Tre kategorier framkom: Behov av att skapa trygghet hos den äldre, Vikten av att organisera vården för den äldre och Vara följsam i kontakten med den äldre. Distriktssköterskorna hade ett brett perspektiv på hälsa: sociala faktorer, kost, motion samt bibehållna vanor vid kognitiv svikt. Teamet och uppföljning anpassades efter behov. Distriktssköterskorna påtalade fördelarna med kontinuitet och fast vårdkontakt för att skapa trygghet, få förtroende och kunna arbeta hälsofrämjande med äldre. 

Slutsats: Distriktssköterskorna såg flera viktiga faktorer för att kunna arbeta hälsofrämjande med de äldre så som kontinuitet, kunskap och ett personcentrerat förhållningssätt.

Abstract [en]

Summary

Background: A healthy population of aging people makes it easier for society to cope. In many Swedish regions health promotion performed by Community HealthCenters includes people up to the age of 70. Persons aged 80 and above aren´t included in this structure. The health of the elderly encompasses many aspects, which can be supported through health promotion and a person-centred approach.

Aim: The aim of this study was t0 examine district nurses’ experiences working with health promotion with elderly over the age of 80 years, in the primary care setting.

Methods: A qualitive study with an inductive approach according to Graneheim and Lundman was conducted. Through a purposive selection twelve district nurses from southern Sweden participated in semi-structured interviews in the autumn of 2024

Results: Three categories emerged: The need to create a sense of safety for the elderly, The importance of organising the care for the elderly and Be flexible in the contact with the elderly. The district nurses had a broad perspective on health: social factors, diet, exercise, maintaining habits in cases of cognitive decline. Team and follows ups were adapted depending on need. Continuity and primary care contact gave the elderly a sense of safety and made it possible to promote health.

Conclusions: The district nurses saw several importande factors in order to work with health promotion with the elderly for example continuity, knowledge and a person-centered approach.
